A “Broken Arrow” is defined as an unexpected event involving nuclear weapons that results in the accidental launching, firing, detonating, theft or loss of the weapon.

US radar operators in Moorestown, New Jersey, alerted NORAD that a nuclear attack would strike Tampa, Florida, at 9:02am. It was quickly discovered that a simulation tape had been playing the same time a satellite appeared on the instruments.
