A “Broken Arrow” is defined as an unexpected event involving nuclear weapons that results in the accidental launching, firing, detonating, theft or loss of the weapon.
A non-nuclear explosion occurred in a Titan II missile silo in Damascus, Arkansas, in the US when a US Air Force repairman dropped a wrench socket. It pierced a missile’s fuel tank leading to the explosion, injuring twenty-two personnel, one of them fatally. The nuclear warhead was discovered intact.
