A “Broken Arrow” is defined as an unexpected event involving nuclear weapons that results in the accidental launching, firing, detonating, theft or loss of the weapon.

A launch control center at Warren Air Force Base, WY lost command of 50 Minuteman III intercontinental ballistic missiles (ICBMs) and with it their ability to detect and cancel any unauthorized launch attempts for nearly an hour.
