A “Broken Arrow” is defined as an unexpected event involving nuclear weapons that results in the accidental launching, firing, detonating, theft or loss of the weapon.

NORAD computers identified a full-scale, pre-emptive attack by the Soviet Union on the US. It turned out to be a false alarm when it was found a technician had loaded a simulation into the system without classifying it as such.
